Burlington Ultimate Winter League Registration 2009-2010

Steps: Please fill out the questionnaire below, accept the waiver, then click on the "Submit..." button to save your information and pay your dues. Once per person please. Questions, concerns, problems? Contact the GMDA Board via email at: gmdaboard@gmail.com.

Registration Period/Capacity: Registration opens on Monday, October 26, 2009 at noon, and it closes on Thursday, November 5, 2009 at 11:59 PM. This is the registration period for BOTH sessions, so don't hold off signing up for the second session. The maximum capacity is 48 women and 96 men per session. Spots will be granted on a first paid basis. This registration form will remain up and active until the November 5th deadline even if maximum capacity is reached before. If maximum capacity is already reached by the time you register, you will be prompted that you are registering onto a waitlist, and you will not be asked to pay just yet. Waitlisted registrants will be contacted by the GMDA Board in order of registration time stamp if/when a spot opens.

When/How you will know what team(s) you're on: The dates of the drafts are November 9, 2009 (for session 1) and February 8, 2010 (for session 2.) The team rosters for session 1 will be posted at indoor pickup at the Fieldhouse on November 10, 2009, and the team rosters for session 2 will be posted at indoor pickup at the Fieldhouse on February 9, 2010. The rosters will also be uploaded on the website under Winter League/Rosters, and you should also receive an email or a call from your captain(s) letting you know you’re on their team.

Ability & Availability Self-Ratings
Please assess yourself seriously and as accurately as possible. Do not expect the volunteers at the draft to know who you are and how you truly rate as a player. Inaccurate ratings can result in unbalanced teams and/or teams with a no-show problem.

Waiver

If I observe any unusual significant hazard while participating, I will remove myself from participating, and bring the hazard to the attention of a staff member of the league.
 
Form Submission & Payment
2009-2010 dues are $75 per session or $130 for both. One price for all, including volunteers and students. Dues can only be paid via Paypal this year due to past non-payment and burden on volunteers. Note: Paying via Paypal does not require a Paypal account.
